Abstract

The Relationship of Self-care and Health Literacy with Mental Health among Citizens of Tabriz City

Abstract:

Background & Objectives: The importance of mental health in personal and social life of people as well as the high cost of health services in today's world necessitates people to attempt for acquiring personal skills for maintaining and improving their health. In this regard, self-care and health literacy, as dynamic and available tools for individuals, can play a decisive role in promoting equity in mental health.  
Methods: This study was a descriptive-analytic study performed on 414 citizens over 15 years of age in Tabriz, Iran who were selected through multi-stage cluster sampling. Data gathering was done using general health questionnaire with three dimensions, literacy questionnaire consisted of five dimensions of reading comprehension, access, understanding, evaluation and decision making and a researcher-made questionnaire about self-care. Data analysis was performed using t-test, Fisher test, Pearson correlation coefficient and multivariate linear regression and through SPSS22 software.
Results: Mean score of mental health was 58.38, mean score of self-care was 54.46 and mean score of health literacy was 77.20. There was a significant and direct relationship between mental health and variables of age, marital status, education level, and self-care.
Conclusion: The present study emphasizes on the importance of training on and accepting responsibility for mental health. People can maintain and improve their health and accept responsibility in this regard through acquiring sufficient knowledge on health aspects.

Cost-effectiveness of Laparoscopic Sleeve Gastrectomy and Laparoscopic Roux-en-Y Gastric Bypass in Patients with Morbid Obesity: A Systematic Review

Abstract:
Background & Objectives: Obesity affects the health of people and imposes costs on them. Since there are two common treatment methods, Roux-en-Y gastric bypass (RYGB) surgery and laparoscopic sleeve gastrectomy (LSG), and there is not enough and aggregated evidence on the cost-effectiveness of these two treatment methods in patients in Iran, therefore, this study was conducted to review the studies on the cost-effectiveness of these treatment methods.
Methods: This is a systematic review study. All articles published in English, which reviewed the cost-effectiveness of these two surgical methods during 2000-2017, were collected from the SCOPUS, PUBMED, MEDLINE, and EMBASE databases. The quality of these articles was evaluated by two researchers based on Grade's guidelines.
Results: The amount of body mass index (BMI) and reduced weight percentage in both methods showed the same effectiveness in weight loss. In Iran, the direct cost of RYGB services was $2991 in the public sector and $4221 in the private sector. For the LSG method, this was $1952 in the public sector and $3,177 in the private sector. ICER for the LSG method compared with the RYGB was $720 and $716 in the private and public sectors, respectively.
Conclusion: Laparoscopic sleeve gastrectomy (LSG) is more effective than the RYGB because it has lower cost and the same effectiveness.

 Determining the Factors Affecting the Length of Hospital Stay Using Hurdle Model

Abstract:
Background & Objectives: There are different statistical methods to identify the factors effective in increasing and decreasing the patients’ length of stay (LOS), each of which having its own advantages and disadvantages. This study aimed to determine the factors affecting LOS in a selected hospital using the available count models and comparing them with the Hurdle model as a new model.
Methods: This descriptive-analytical study was conducted using a retrospective cross-sectional design in a selected hospital in Isfahan in 2019. Using Poisson and Hurdle (binomial and Poisson) count models, the effect of demographic factors of the hospitalized patients and the reasons for visiting the hospital on LOS was analyzed. The goodness of fit of the models and their comparison were performed using deviance statistic.
Results: The mean of LOS was 8.89 days and the median of LOS was 3 days. From among the three models, the binomial Hurdle model was determined appropriate in which, the constant and variables such as age, social security insurance and uninsurance, hospital emergency visits and 115 emergencies, and reasons for visiting psychiatric, orthopedic, and ophthalmic wards were significant. By changing the type of insurance from the reference of other insurances to social security insurance, the mean of LOS decreased by 0.088 units and for uninsured patients, this decrease was by 0.539 units.   
Conclusion: Considering the values of the deviance statistic for Poisson, Hurdle Poisson and negative binomial Hurdle models, the negative binomial Hurdle model is the most appropriate for analyzing these data.
