Abstract

Readiness of Teaching Hospitals to Attract Medical Tourists Based On Organization-Oriented Standards of Joint Commission International (JCI):
a case study

Abstract:
Background & Objectives: The ability to attract tourists in international health markets depends on the quality standards. The aim of this study was to determine the state of educational hospitals of Shahid Beheshti University of Medical Sciences based on organization-oriented standards of joint commission international.
Methods: This Descriptive study was performed in four selected teaching hospitals of Shahid Beheshti University of Medical Sciences in Tehran. Data collection was carried out using  the questionnaires of the organization-oriented standards of joint commission international including 76 questions in 6 areas of improving quality and patient safety, infection prevention and control, leadership and guidance, safety and facility management, training and staff competence and information and communication management. Descriptive statistics were used as mean and percentage. Data were entered into SPSS20 software.
Results: The compliance rate of organization-oriented standards in the hospitals was 81.2% that indicated the relative readiness with regard to the standards of joint commission international in attracting medical tourists. The highest compliance rate of standards was related to safety and facility management (84.4%) and the lowest compliance rate was related to quality improvement and patient safety (75.2 %). 
Conclusion: Hospitals should pay more attention to the quality improvement and patient safety in order to meet international standards and obtain an international accreditation certificate, and they are supposed to improve standards in this area through corrective actions.

  The Effect of Job Success Training on Affective Capital and Flow at Work in Hemophilic Patients

Abstract:
Background & Objectives:
One of the areas where hemophilic patients need help to improve their affective health is in strengthening their affective capital and flow at work experiences. The aim of this study was to determine the effect of job success training on affective capital and flow at work of hemophilic patients.
Methods: The present study was an interventional, descriptive-analytic study. In the first phase, 20 hemophilic patients selected through random sampling were divided into the two groups of experimental and control groups (n= 10). The experimental group received job success training package in eleven 2-hour sessions. The members of both groups were asked to complete the affective capital and the flow at work experiences questionnaires before the intervention, after the intervention and in a follow up, 45 days after the intervention. Data analysis was performed using repeated measures analysis of variance and through SPSS 21.
Results: Mean age of participants was 32.4 years. There was a significant difference between experimental and control groups in affective capital and flow at work experiences in both the post-training phase and follow-up phase. This means that the job success training package was effective on the affective capital and the flow at work experiences for experimental group.
Conclusion: The job success training package can be used to improve the affective capital and flow at work of hemophilic patients and consequently the capacity of these patients.

Abstract:
Background & Objectives:
One of the most valuable assets of any organization is its brand because it causes more profit margins, better cooperation channels and other benefits for the organization. The aim of this study was to investigate the factors affecting brand identity of hospitals in a semi- governmental hospital in Tehran/ Iran.   
Methods: In this descriptive-analytic study, 403 hospitalized patients in a semi-governmental hospital were selected by available sampling method in 2016. The data gathering tool was a researcher-made questionnaire based on the Kapferer model (including 30 questions in 6 domains of brand personality, brand physique, brand culture, relationship, self-image and reflection). Data were analyzed using one-sample t-test in SPSS 16 software.
Results: The highest mean scores were attributed to the importance of service quality (4.21), known cases (4.15) and easy access (4.02). The least mean scores belonged to patients' mental and psychological needs (2.31), the distinctive character (2.45) and happy and motivated staff (2.63). There was a significant relationship between total mean score (3.51) and brand personality, brand physique and brand culture (P<0.05). 
Conclusion: Hospitals cannot survive without attention to the patients' demands. Therefore, by implementing any of the factors affecting the hospital brand identity, it is possible to improve the hospital brand status in the minds of the patients as a superior and valuable brand.

Abstract:
Background & Objectives:
Infant mortality rate is one of the main indicators in the assessment of community health. Around 65% of deaths in children under one year of age is related to the infancy period and undoubtedly the first step in promoting this index is to identify the causes of mortality at this period of life. The present study was conducted to determine the incidence rate and causes of infant mortality in the Neonatal Intensive Care Unit of Shahid Sadoughi Hospital in Yazd/ Iran.
Methods: This descriptive-analytic study was performed on 243 infants admitted to the neonatal intensive care unit of a selected educational hospital in Yazd from 21st March 2006 to 21st March 2007. A checklist was used for data collection. Data were analyzed using ANOVA and Chi-square tests and through SPSS 19.
Results: From 243 studied infants, 39 ones (16.4%) died. Prematurity was the first cause of neonatal death (56.4%) followed by cardiac arrest (47.5%). Neonatal death showed significant reverse relationship with infant’s weight and age at admission and maternal gestational age (P =<0.0001).
Conclusion: Mother and child care should be given more importance in health programs and policies and measures should be taken to prevent the probable factors affecting infant death.

Evaluation of Burnout among Primary Health Care Providers in Markazi Province

Abstract:
Background & Objectives: Nowadays, burnout as a psychological syndrome is a major problem, especially in health services, that has been identified in a large number of health care providers. The aim of this study was to determine the extent of burnout and the relationship between demographic characteristics and burnout dimensions among primary health care providers.
Methods: This cross-sectional descriptive-analytic study was conducted in 2019. The statistical sample included all primary health care providers (n=335) working in one of the medical colleges in Markazi province. Data were collected using Maslach Burnout Inventory (MBI). Simple and multiple regression tests were used. Data were entered into SPSS version 23 software.
Results: The mean score of emotional exhaustion, depersonalization, and personal accomplishments was 15.5, 4.9, and 33.8, respectively. Among health care providers, 21.8% had high emotional exhaustion, 23%, high depersonalization, and 47.5% had low personal accomplishments. The results of multiple regression tests indicated a significant relationship between gender, work experience, and place of service with burnout.
Conclusion: Some strategies such as conducting training courses, providing advisory services, and optimizing human resources management, especially for women, urban employees, and those with high work experience, are suggested to reduce burnout.

Abstract:
Background & Objectives: Ethical values act as the most powerful factors influencing citizenship behaviors. This research was conducted to investigate the relationship between ethical climate and organizational citizenship behavior in nurses of hospitals affiliated to Shahid Beheshti University of Medical Sciences.
Methods: This descriptive-analytical study was performed on 307 nurses selected through random sampling in 2016. Data gathering tools were two questionnaires of ethical climate and organizational citizenship behavior. Data analysis was done through SPSS20 and using descriptive statistics, t-test, One- Way ANOVA, correlation and multiple regression tests.
Results: Mean scores of ethical climate and citizenship behavior were respectively 3.51 ±0.55 and 3.49 ± 0.45 that reflects the favorable view of nurses towards ethical climate and citizenship behavior. Head nurses had the highest mean scores in the dimensions of ethical climate. Among the dimensions of citizenship behavior, the dimension of conscience gained the highest mean score among nurses. There was a significant relationship between employment status and ethical climate (P<0.001) and also a positive and significant correlation between ethical climate and citizenship behavior (P<.0.001).
Conclusion: Promotion of ethical climate enables nurses to respond better to ethical stress, to adapt to other causes of workplace dissatisfaction and consequently showing citizenship behaviors. Some meetings are required to be held in order to improve the relationship between physicians and nurses, to create alignment and understanding among staff and consequently to improve citizen behaviors.

Abstract

Abstract:
Background & Objectives: Ensuring high quality care for diabetic patients is a challenge for healthcare systems throughout the world. The aim of this study was to evaluate the relationship of quality of services provided to type 2 diabetic patients with self- assessment in comprehensive health service centers.
Methods: This descriptive-analytic cross-sectional study was conducted on 203 patients with type 2 diabetes referred to ten comprehensive health service centers of south Tehran/ Iran in 2018 selected through convenience sampling method. Data collection tool was a questionnaire for comprehensive assessment of quality of health services. Data were analyzed through SPSS22 and using t-test, ANOVA and logistic regression statistical tests.
Results: From the perspective of patients, mean score of quality of services was 7.47. Among different health services, the highest mean score of quality was related to trust and continuity of service while, support and prevention gained the lowest mean score of quality. There was a significant statistical relationship between the quality of service and self-assessment of diabetes control in the past 12 months (P= 0.007). Also, a significant relationship was found between the quality of service and diabetes complication (P = 0.04). With each unit increase of the quality of service, the odds of a standard visit increased by 27% (P = 0.013). 
Conclusion: Design and implementation of interventions in order to increase the social support of patients and sharing their experiences with each other as well as improving the dimensions of prevention and safety in care are essential.

Abstract:
Background & Objectives: Creating and developing health-promoting behaviors in middle-aged women has a potential impact on promoting their health and quality of life. Therefore, it is necessary to design appropriate educational interventions in this group. The objective of this study was to determine the effect of education based on health belief model on health-promotion behaviors in middle-aged women.
Methods: This descriptive-analytical study was performed as an intervention in 2019 on 64 middle-aged women referring to two comprehensive health services centers in Mashhad. Data collection tools were a standard questionnaire of health-promotion behaviors and a researcher-made questionnaire based on the health belief model that was completed before and after training in two intervention and control groups. Independent t-test, paired t-test and Mann-Whitney U test were used.
Results: Before the intervention, the two control and intervention groups were identical in terms of the mean overall score of health-promotion behaviors, however, one month after the intervention, the mean overall score of health-promotion behaviors and its dimensions in the intervention group was significantly higher than that in the control group.
Conclusion: Education based on the health belief model can improve health-promotion behaviors. Thus, it is necessary for health care providers in health centers to be trained on using models of behavior change.
