Abstract

Survey of under 5 year's children injury and accident’s mortality causes in Kerman state-2013

 Introduction: One of the most important improvement and health indexes is under 5-year children’s mortality. Recently under 5-year children decreased but it is still more than developing countries. This research aimed to study accidents and injuries of under 5-year children in province of Kerman.

Method: This was a descriptive analytical prospective study in2013. This project included 1303 under 5 years children who could refer to three training hospitals 'emergencies which data on accidents for a period of 1 year (2013) were collected from record accident and mortality of health military forms of Kerman Medical University (‌. The study was conducted in 1303 under 5-year children. Data were analyzed by spss19 software and using t-test, ANOVA, 2&chitests.

Results: Among 1303 under 5-year children were studied, 159 people died. 69% were girls and 31% were boys. The most accidents that occurred for them were injury, poisoning and falling. Accidents range were higher among girls and most of them occurred in 3-4 years old children. Age, Sex and season of death did not have correlation with kind of accident however there was a significant correlation between accidents occurred in under 5-year children and season of death.

Conclusion: Three main causes of accidents occurring were injury, poisoning and falling so we must choose proportional trainings with age group and accidents that happened for them, and in addition we should try more to train all society particularly parents that care children.

The Role of Emergency Medicine Specialist on Performance of the Emergency Department of Kerman Shahid Bahonar Hospital: A Qualitative Study

Abstract:

Introduction: Emergency department is the important part of the healthcare system. Today, the need for integrated emergency care services and delivering qualified care services have been felt more than before. The aim of this study was to investigate the role of emergency medicine specialists on performance of the emergency department of Shahid Bahonar hospital, Kerman.

Methods: This phenomenological qualitative study was conducted on 16 emergency medicine specialists (EMSs) and nurses of emergency department and Emergency Operations Center (EOC) who were selected using purposeful sampling in 2015. Data were collected through semi-structured interviews and analyzed using framework analysis.

Results: The main themes included economics impact, service quality promotion, and emergency management. Some sub-themes included hospitals revenue improvement, tariffs definition for emergency services, effective treatment, improvement of emergency team knowledge, emergency department management, and triage quality improvement.

Conclusion: With presence of EMS in healthcare system, the quality of healthcare delivery system has been improved and waiting time for healthcare services has been significantly decreased. By decreasing waiting time, determining of patient statue and determining the ward for patient in the hospital, emergency beds for other possible admissions and emergency preparedness for future admissions will be kept empty.

The Impact of Accreditation on the Performance of Hospital Emergency Departments

Abstract:

Background & Objectives: Accreditation, as a formal external evaluation system, has a critical role in improving quality, safety and effectiveness of hospital services.  The aim of this study was to evaluate the effects of accreditation standards on the performance of Accident and Emergency (A&E) departments of hospitals in Sari, Iran.
Methods: This cross-sectional descriptive - analytic study was carried out in eight hospitals in Sari, Iran. Information related to the accreditation scores and five performance indicators of A&E departments (i.e., average triage time, percentage of cardiopulmonary resuscitation failure, percentage of patients who were decided upon within 6 hours, percentage of patients sent out of the A&E department within 12 hours and percentage of patients left A&E department against the medical advice) in (March 2013 and March 2014) and (March 2014 and March 20115) were gathered form hospitals and accreditation office of Mazandaran University of Medical Sciences. Data were analyzed through SPSS (V.22) software and using descriptive and inferential statistics.
Results: The mean scores of accreditation of A&E departments in 2013 and 2014 were 76% and 62.2% respectively. The differences between performance indicators of A&E departments in 2013 and 2014 were not statistically meaningful. There was no significant relationship between accreditation and the performance of A&E departments of hospitals.
Conclusion: Accreditation did not improve A& E performance in Sari hospitals. Hence, there is a pressing need for improving hospital accreditation system including standards, methods and surveyors. Besides, hospital managers should use internal quality management systems to complement and strengthen the impact of external accreditation system in improving key performance indicators of hospitals.

Abstract:
Background & Objectives: Correct relationship between nurses and patients will improve the therapeutic outcomes and improve the patient's recovery process. One of the most important items in improving communication skills, as an essential part of providing nursing services, is the attitude and feedback of nurse to various phenomena and to create the necessary internal preparation for making positive changes in cognitive domain. This study aimed to investigate the effect of participatory teaching of positive thinking skills on communication skills of nurses in the emergency department of a selected hospital in Kerman city.
Methods: This analytical study was an interventional study and it was conducted on 60 nurses at the selected educational hospital divided into the two groups of experimental and Control. Queendom Communication Skills Questionnaire was used for data collection. The experimental group received positive thinking skills education during 9 sessions. Independent t-test, ANOVA and single-variable covariance analysis test were used for data analysis. Data were entered into SPSS version 22.
Results: Mean of total score of communication skills increased in the experimental group. Based on the post-test results, there was a significant improvement in the mean scores of listening skills, the ability to receive and send messages, emotional control, connection with decisiveness and insight into the communication process of nurses  based on their group (P <0.05).
Conclusion: Considering the effectiveness of teaching positive thinking skills in increasing nurses' communication skills, it is recommended to provide training for these skills from the beginning of nursing students’ education.

Abstract:
Background & Objectives: Patients’ length of stay in emergency departments is one of the key criterion which has been focused by hospital managers recently. This study aimed to assess the influencing factors on patients’ length of stay in an emergency department of a training hospital affiliated by Alborz Medical University of Medical Sciences.
Methods: This was a descriptive-analytical study with a cross-sectional design in 2017. Study population consisted of all inpatients in a six month time period from the beginning of the study year. Through the use of Cochrane sample size calculation formula 555 numbers of samples were selected which their related data were obtained from the review of patients’ medical records. To test study hypotheses data were entered to a SPSS version 20 and analyzed through descriptive statistical analysis method, Pearson correlation test, t-test, and ANOVA.
Results: Mean of patients’ length of stay was 11.31 hours. Evidence showed that this mean time was correlated with some of the factors including internal or external medical counseling, the necessity to have a medical laboratory test, a medical imaging test or the time interval between triage and the first physician’s visit.
Conclusion: Modifying hospital processes and performance procedures in an emergency department with an aim to improve service quality and effective interactions among different hospital units can play a helpful role in solving existing problems.
