fa شناسایی و ارزیابی خطاهای انسانی در بخش اندودونتیکس کلینیک دانشکده دندانپزشکی دانشگاه علوم پزشکی قزوین با استفاده از تکنیک SHERPA Identification and Assessment of Human Errors Using SHERPA in the Endodontic Department of Clinic of Dentistry Faculty, Qazvin University of Medical Sciences پژوهشي Research <div style="text-align: center;">شناسایی و ارزیابی خطاهای انسانی در بخش اندودونتیکس کلینیک دانشکده دندانپزشکی دانشگاه علوم پزشکی قزوین با استفاده از تکنیک SHERPA مجتبی جعفروند۱، حدیث خوشنواز۱، سمیرا کاظمی۱، سکینه ورمزیار۲*، معصومه قربانی‌ده۱</div> <div> ۱. دانشجوی کارشناسی ارشد، کمیته تحقیقات دانشجویی، دانشکده بهداشت، دانشگاه علوم پزشکی قزوین، قزوین، ایران ۲. استادیار، گروه مهندسی بهداشت حرفه‌ای،‌ دانشکده بهداشت، دانشگاه علوم پزشکی قزوین، قزوین، ایران * نویسنده مسئول: قزوین، بلوار شهید باهنر، دانشگاه علوم پزشکی قزوین، دانشکده بهداشت تلفن: ۳۳۳۵۹۵۰۲-۰۲۸                                  Email: Svarmazyar@qums.ac.ir                        چکیده: </div> <div style="text-align: justify;"> مقدمه: واحد اندودونتیکس در حرفه دندانپزشکی از حساسیت بالای برخودار بوده و وجود خطای انسانی به صورت آشکار و پنهان، سلامتی بیمار و دندانپزشک را به خطر می‌اندازد. هدف از مطالعه حاضر، شناسایی و ارزیابی خطاهای انسانی بخش اندودونتیکس کلینیک دانشکده دندانپزشکی دانشگاه علوم پزشکی قزوین با استفاده از تکنیک SHERPA بود. روش بررسی: این مطالعه توصیفی به صورت مقطعی در سال ۱۳۹۵ انجام گرفت. تعداد خطاها در هر وظیفه، با استفاده از برگه‌کار SHERPA که شامل کد وظیفه، وظیفه شغلی، نوع خطا، توصیف خطا، پیامد خطا و سطح ریسک بود، به دست آمد. ابتدا با استفاده از روش تحلیل سلسله مراتبی وظیفه، مراحل انجام کار در واحد به وظایف و زیر وظایف خود تقسیم و ۹ وظیفه بحرانی تعیین گردید. سپس با استفاده از تکنیک SHERPA انواع خطاها، شناسایی و ریسک آن‌ها ارزیابی شد.  یافته‌ها: در مجموع ۱۴۸ خطای انسانی در بخش اندودونتیکس شناسایی شدند. بیشترین میزان خطا در وظیفه معاینه و گرفتن شرح حال با ۳۵ خطا و کمترین میزان خطا در وظیفه بی‌حس کردن با ۷ خطا بود. در بین تمامی وظایف اصلی بخش اندودونتیکس، بیشترین خطا از نوع عملکردی در وظیفه طرح درمان (٩/٤٥%) بود. همچنین٥٢/٦٣% خطاها از نوع عملکردی با سطح ریسک نامطلوب ارزیابی شدند. بحث و نتیجه‌گیری: نتایج حاصل از این مطالعه نشان داد که برای کاهش وقوع خطاها در حرفه دندانپزشکی لازم است راهکارهای کنترلی نظیر استفاده از افراد با مهارت و تجربه کافی، تناسب بین تعداد بیماران و متخصص، برنامه کار و استراحت و تدوین دستورالعمل‌ها صورت گیرد.   واژگان کلیدی: اندودونتیکس، خطای انسانی، تکنیک SHERPA، دانشکده دندانپزشکی   ارجاع: جعفروند مجتبی، خوشنواز حدیث، کاظمی سمیرا، ورمزیار سکینه، قربانی‌ده معصومه.  شناسایی و ارزیابی خطاهای انسانی در بخش اندودونتیکس کلینیک دانشکده دندانپزشکی دانشگاه علوم پزشکی قزوین با استفاده از تکنیک SHERPA. مجله پژوهش‌های سلامت محور ۱۳۹۶؛ ۳(۳): ۲۷۶-۲۶۷.</div> <div>               </div> <div style="text-align: center;">Identification and Assessment of Human Errors Using SHERPA in the Endodontic Department of Clinic of Dentistry Faculty, Qazvin University of Medical Sciences   Jafarvand Mojtaba1, Khoshnvaz Hadith1, Kazemi Samira1, Varmazyar Sakineh2*,  Ghorbanideh Masumeh1</div> <div> 1. Methods: This descriptive and cross-sectional study was conducted from 26/4/2017 to 21/8/2017. The number of error per task was obtained using worksheet of SHERPA including: task code, job task, error type, error description, error consequence and risk level. At first, using hierarchical task analysis, steps of doing work in the unit were divided into tasks and sub-tasks and nine critical tasks were determined. Then, using SHERPA technique, types of errors were identified and their risk was evaluated. Results: A total of 148 human errors were identified in the Endodontic Department of Clinic. The highest error rate was in the tasks of examination and history taking with 35 errors, and the lowest error rate was on the task of anesthetizing with 7 errors. Among all the main tasks, the most frequent errors were of functional type at the task of the treatment plan (9.45%). Also, 52.63% of errors were related to the functional errors with an undesirable risk level. Conclusion: The results of this study indicate that in order to reduce the occurrence of errors in the dentistry profession, it is necessary to take control measures such as using competent people, adjusting the number of patients with the number of specialists, balancing work hours and rest, and developing guidelines.   Keywords: Endodontic, Human error, SHERPA technique, Dentistry</div> <div>  Citation: Jafarvand M, Khoshnvaz H, Kazemi S, Varmazyar S, Ghorbanideh M.
