fa بررسی علل خطاهای دارویی و عدم گزارش‌دهی آنان از دیدگاه پرستاران: یک مطالعه موردی Causes of Medication Errors and Barriers of Error Reporting in point of views of Nurses: a case study پژوهشي Research <div style="text-align: center;">بررسی علل خطاهای دارویی و عدم گزارش‌دهی آنان از دیدگاه پرستاران: یک مطالعه موردی</div> <div style="text-align: center;">مهناز مایل افشار‌۱*‌، مهدی معمارپور۲  </div> <div style="text-align: justify;">۱. دانشجوی دکترا، گروه مدیریت خدمات بهداشتی- درمانی، دانشگاه آزاد اسلامی، واحد علوم و تحقیقات تهران، تهران، ایران ۲. دانشجوی دکترا، گروه مهندسی صنایع، دانشگاه آزاد اسلامی، واحد علوم و تحقیقات تهران، تهران، ایران * نویسنده مسئول: تهران، خیابان ولیعصر ابتدای نیایش، مرکز آموزشی تحقیقاتی و درمانی قلب و عروق شهید رجایی تلفن: ۲۳۹۲۳۱۳۶ - ‌ ۰۲۱                               Email: mahnazmayelafshar@yahoo.com             چکیده:</div> <div style="text-align: justify;">مقدمه: دارو رایج‌ترین کالای درمانی مصرفی در واحدهای ارائه کننده خدمات سلامت است. اجرای صحیح دستورات دارویی، بخش مهمی از فرآیند درمان و مراقبت بوده که جزء اصلی عملکرد پرستاران و کادر درمان می‌باشد و  بروز اشتباهات دارویی می‌تواند باعث بروز مشکلات جدی در سلامت شده و تهدیدی برای ایمنی بیماران محسوب گردد. این پژوهش با هدف بررسی علل خطاهای دارویی و عدم گزارش‌دهی از دید پرستاران انجام شد. روش بررسی: مطالعه حاضر از نوع توصیفی- تحلیلی بود که به صورت مقطعی با استفاده از گزارش‌های خطاهای دارویی که به واحد اعتباربخشی بیمارستان منتخب در بازه زمانی ۱۳۹۰-۱۳۹۶ ارسال شده بودند و نیز تکمیل پرسشنامه توسط ۱۰۲ پرستار که به روش نمونه‌گیری تصادفی چندمرحله‌ای انتخاب شده بودند، در یک بیمارستان تخصصی قلب در شهر تهران انجام شد. از آزمون‌های آماری من ویتنی و کروسکال - والیس استفاده شد. یافته‌ها: مهم‌ترین علل موانع گزارش‌دهی خطاهای دارویی از دیدگاه پرستاران به ترتیب اهمیت، شلوغی و ازدحام بیماران بخش (٢٩/٦%) ترس از تنبیه انضباطی (٢٧/٧%) بود. در ایجاد خطاهای دارویی نیز خستگی زیاد ناشی از کار اضافی پرستار (٤٠/٢%)، تراکم زیاد کار و کمبود تعداد پرستار نسبت به بیمار در بخش (٣٨/٢%) از مهم‌ترین دلایل عنوان شدند. بحث و نتیجه‌گیری: به‌کارگیری یک برنامه آموزشی مناسب توسط مدیران حوزه سلامت جهت توجه به فرهنگ ایمنی بیمار و همچنین  توانمندسازی پرستاران می‌تواند در کاهش میزان خطا مؤثر باشد.   واژگان کلیدی: خطاهای دارویی، ایمنی دارو‌، موانع گزارش‌دهی، بیمارستان تخصصی، بیمارستان آموزشی، پرستاران   ارجاع: مایل افشار‌ مهناز‌، معمارپور مهدی. بررسی علل خطاهای دارویی و عدم گزارش‌دهی آنان از دیدگاه پرستاران: یک مطالعه موردی. مجله پژوهش‌های سلامت محور ۱۳۹۷؛ ۴(۳): ۵۹-۲۴۵.</div> <div style="text-align: justify;">            </div>   <div style="text-align: center;">Causes of Medication Errors and Barriers of Error Reporting in point of views of Nurses: a case study   Mayelafshar Mahnaz1*, Memarpour Mehdi2  </div> <div style="text-align: justify;"> 1. Rajaie Cardiovascular Medical and Research Center, Iran University of Medical Sciences, Tehran, Iran Tel: 009823923136                                                   Email: mahnazmayelafshar@yahoo.com Abstract: Background & Objectives: Medication is the most commonly used therapeutic product in health care centers.The correct administration of medication orders is an important part of the treatment and care process, which is the major part of responsibility of nurses and medical staff. The occurrence of medication errors can cause serious health problems and threaten the safety of patients. This study was done to investigate the factors associated with nurses' medication errors and their refusal to error reporting in points of views of nurses. Methods: The present study was a descriptive-analytic study that was done cross-sectionally using error reports sent to the accreditation unit of a specialty cardiac hospital in Tehran during 2012-2018. Moreover, 102 nurses selected through randomized multistage sampling method were asked to complete a questionnaire. Mann-Whitney and Kruskal-Wallis statistical tests were used for data analysis. Results: According to the nurses’ point of views, the most important barriers of error reporting were respectively crowded wards (29.6%) and fear of punishment (27.7%) and causes of medication errors were respectively high fatigue due to the nurses’ overworking (40.2%), high workload and shortage of nurses in comparison to the number of patients (38.2%). Conclusion: Use of an appropriate training program to estabilish the culture of paying attention to the patients’ safety as well as empowering the medical staff in this regard can be effective in reducing medication errors rate.   Keywords: Medication errors, Medication safety, Reporting barriers, Specialty hospital, Educational hospital, Nurses   Citation: Mayelafshar M, Memarpour M.
