line-height: 107%; font-family: 'B Nazanin';">چکیده</span></b></p> <div> <p dir="RTL" style="text-align: justify;"><strong><em>مقدمه:</em></strong> بیمارستان‌ها به عنوان یکی از پر هزینه‌ترین سازمان‌های ارائه خدمات بهداشتی -درمانی مطرح می‌باشند. در این مطالعه نسبت به شناسایی عوامل مؤثر بر تولید هزینه‌های بستری حول ابعاد سه گانه نظام سلامت شامل دریافت کنندگان، ارائه دهندگان مراقبت بهداشتی و سازمان های بیمه‌گر اقدام گردید.</p> <p dir="RTL" style="text-align: justify;"><strong><em>روش بررسی:</em></strong>این مطالعه از نوع کیفی بود که به شیوه پدیدارشناسی انجام گردید. در این پژوهش ١٢ نفر از صاحب نظران و کارشناسان خبره در زمینه مدیریت و کنترل هزینه‌های بیمارستان که به روش هدفمند و گلوله برفی انتخاب شده بودند‌، شرکت نمودند. اطلاعات با استفاده از بحث گروهی متمرکز جمع‌آوری شد. به منظور تحلیل داده&rlm;ها از روش تحلیل چارچوبی استفاده گردید.</p> <p dir="RTL" style="text-align: justify;"><strong><em>یافته‌ها:</em></strong> عوامل مؤثر بر تولید هزینه‌های بستری در قالب سه کد اصلیدریافت کنندگان، ارائه دهندگان خدمات درمانی و سازمان‌های بیمه‌گر طبقه بندی گردید. فقدان دانش و آگاهی لازم و کمرنگ بودن اهمیت خود مراقبتی به عنوان مهم‌ترین کدهای فرعی تشکیل دهنده کد اصلی فرد بیمار و عوامل ضعف مدیریت و نظارت، بستری‌های طولانی مدت بیمار در دوران نقاهت، عدم اجرای برنامه پزشک خانواده، تعهدات ضعیف و مشکلات ساختاری سازمان‌های بیمه‌گر نیز به عنوان مهم‌ترین کدهای فرعی مرتبط با کدهای اصلی ارائه‌دهندگان مراقبت و سازمان‌های بیمه گر شناسایی گردید.</p> </div> <p style="text-align: justify;"></p> <p dir="RTL" style="text-align: justify;"><strong><em>بحث و نتیجه‌گیری:</em></strong> با ارتقاء و بهبود برنامه‌های خودمراقبتی، سلامت محور نمودن مراقبت‌های ارائه شده، اجرای برنامه پزشک خانواده در کنار ایجاد مراکز درمان بستر، تخصصی نمودن مدیریت بیمارستان‌ها، بهبود کمیت و کیفیت نظارت‌های اعمال شده و اصلاح ساختار کارشناسی سازمان‌های بیمه‌گر می‌توان نسبت به مدیریت هزینه‌های بستری اقدام نمود.</p> <p dir="RTL" style="text-align: justify;"></p> <p dir="RTL" style="text-align: justify;"><strong><em>واژگان کلیدی:</em></strong> هزینه بستری،بیمار، ارائه دهندگان خدمات درمانی، سازمان بیمه‌گر، بیمارستان دولتی، مطالعه کیفی</p> <p dir="RTL" style="text-align: justify;"></p> <p dir="RTL" style="text-align: justify;"><strong>ارجاع:</strong> جعفری محمد، لشکری مرضیه، محمودی فاطمه، دهنویه رضا، نوری حکمت سمیه، ایزدی آذر.شناسایی عوامل مؤثر بر تولید هزینه‌های بستری در بیمارستان‌های دولتی: یک مطالعه کیفی. مجله پژوهش‌های سلامت محور ١٣٩٥؛ ٢(١): ٥٤-٣٩.</p> <p dir="RTL"></p> <p align="center"><strong>Effective Factors in Hospitalization Costs of the Public Hospitals: A Qualitative Study</strong></p> <p align="center"></p> <p align="center">Jafari Mohammad¹, Lashkari Marzieh²,Mahmoudi Fatemeh³, Dehnavi Reza⁴, NooriHekmat Somayeh⁴, Izadi Azar⁵<span dir="RTL">*</span></p> <p align="center"></p> <p align="center">Ÿ Received: 17. 01. 2016&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; In this study, identifying factors influencing hospitalization costs based on the three aspects of the health system including patients, health care providers, and insurance organizations were considered.</p> <p style="text-align: justify;"><strong>Methods:</strong>This was a qualitative and phenomenological study. In this study, 12 experts in the field of management and controlling hospital&rsquo;s costs were selected through purposive and snowball sampling method. Data were collected using focused group discussion. Framework analysis was used for data analysis.</p> <p style="text-align: justify;"><strong>Results:</strong>The factors affecting hospitalization costs were classified into three codes including patients, health care providers, and insurance organizations.
